Please review this list and check off any items which concern you about your pet's well-being. This checklist may serve to remind you of problems you may have noticed with your pet. It will also help the doctor better evaluate your pet's health. ______ 1. My pet itches excessively and has a dry or flaky coat. ______ 2. My pet sheds excessively. ______ 3. My pet has had a flea or tick problem in the last year or two. ______ 4. My pet has unacceptable body odor. ______ 5. My pet has bad-smelling breath and/or red, irritated gums. ______ 6. My pet has frequent episodes of vomiting and diarrhea. ______ 7. My pet has lost or gained weight. ______ 8. My pet is drinking more water than usual. ______ 9. My pet urinated more frequently and has accidents. ______ 10. My pet sometimes seems stiff or has difficulty getting up and down or is sore after running and jumping. ______ 11. My pet has been coughing or seems winded after walking or playing, or has excessive panting. ______ 12. My pet's behavior or personality has changed recently. ______ 13. My pet has new or unusual growths or lumps.
